Lehigh Valley Home Care allows you the comfort of both. You are connected to a first-class regional hospital in the comfort of your own home.

What is home care?

Home care services are available when you don’t need to be in the hospital, but you still need medical attention. Our team of health care professionals includes nurses, home health aides, social workers and rehabilitation therapists who help you recover as quickly as possible.

You will learn how to monitor and care for yourself. We believe you will heal faster and better if you are comfortable in your environment and involved in your care.

What kind of care does home care offer?

Nurses provide:

  • Diabetes care and education
  • Diet instruction and nutritional support
  • Dressing (bandage) changes
  • Injections
  • Intravenous therapy
  • Medication monitoring
  • Pain management
  • Patient and family education
  • Patient assessment and monitoring
  • Behavioral health

  • Cardiac care
Home health aides provide:
  • Bathing and personal hygiene
  • Dressing
  • Meal preparation
Medical social services include:
  • Counseling and education regarding long-term planning and available community resources
  • Short-term counseling to assist with adjustment to illness and use of support services
Rehabilitative therapies include:
  • Physical therapy to help with balance, movement, strength, prosthetics and walking
  • Occupational therapy to help with activities of daily living
  • Speech and language therapy to improve speech, language and cognitive function as well as swallowing problems

Who is eligible for home care?

To receive home care services you must be under the care of your physician and meet the requirements of your insurer. Your physician or the hospital’s discharge planner may refer you, or may call us directly.

Who pays for home care?

Eligible home health services generally are covered by Medicare and Medical Assistance as well as most private insurance plans.
